Social Prescription Link Workers – Voluntary Action South West Surrey

Social Prescribing involves helping people who have been referred or “prescribed” by their GP, Adult Social Care or other health professional to social, recreational and non-medical support within the community to help them improve their wellbeing.

Voluntary Action South West Surrey is currently recruiting two Link Workers one for Guildford and one for Waverley to deliver a social prescription service.

This is a new service and you will be responsible for the setup and then the day-day coordination and delivery of the service, from receiving referrals, carrying out assessments, linking the person to appropriate community services or activities and then following up to ensure the referral was successful. You will also be responsible for developing a small team of volunteers to help deliver the service.

To be successful in this role you will need to have a support or advice background, have an understanding of the issues that our client group maybe facing and be able to work with a wide range of people in an empathetic and non-judgemental way. You will be organised, able to work independently and manage your time well to ensure you are able to meet challenging workloads. You will need to be a confident IT user.

The role is subject to an Enhanced DBS check.

The salary is £20,000 per annum for a 28 hour week. The roles are based in central Guildford.
