Social Prescribing involves helping people who have been referred or “prescribed” by their GP, Adult Social Care or other health professional to social, recreational and non-medical support within the community to help them to improve their health and wellbeing.
We require an additional Link Worker to join our team of five. The role involves receiving referrals, carrying out assessments and then linking the person referred to appropriate community services or activities, following up and offering support as appropriate.
To be successful in this role you will need to have experience of working with people in a support role, experience of advice work would be an advantage. You will need to have an understanding of the issues that people referred maybe facing and be able to work with a wide range of people in an empathetic and non-judgmental way. You will be organised, able to work independently and manage your time well to ensure you are able to meet challenging workloads. You will need to be a confident IT user.
The role is subject to an Enhanced DBS check.
The salary is £26,220 pa for full time hours (37.5 hrs a week) Part time working will be considered. The roles are based in central Guildford, travel throughout Guildford and Waverley will be required.
