Guildford and Waverley Social Prescribing
Self-Referral Form

Social Prescribing aims to support people to take better control over their health and wellbeing. It is designed for individuals with a range of social, emotional or practical needs who need help navigating their way through the community services and voluntary organisations available locally. The Social Prescribing Link Worker will discuss your concerns through one-to-one appointments, either telephonically, virtually, in a GP Surgery or in a Community setting. You can register yourself, or make a referral on behalf of someone else, with their consent. When you have completed the form, a member of our team will aim to be in touch within 10 working days.

Guildford and Waverley Social Prescribing Service

Self-Referral Form

The Guildford and Waverley Social Prescribing Service is hosted by Voluntary Action South West Surrey. We are currently trialling the use of self-referrals and as such the self-referral pathway is currently only open to patients registered to a GP practice in the East Guildford Primary Care Network. These practices are:

  • Austen Road Surgery

  • Horsley Medical Practice

  • Merrow Park Surgery

  • Shere Surgery

  • St Luke's Surgery

  • The Villages Medical Centre

Inclusion Criteria

The Social Prescribing Service is for anybody over the age of 18 registered to a GP in the stated Primary Care Network(s) above, but particularly for:

  • People with non-medical needs.

  • People who would benefit from proactive, preventative support to maintain or improve their health and wellbeing.

  • People who are lonely and or isolated, particularly people who have managed long term mental health conditions or who suffer chronic pain.

  • People who are experiencing low-level mental ill-health.

  • People with managed mental health diagnoses who may need additional support to access social and community opportunities to improve their quality of life.

  • Older people who are at risk of reducing independence.

  • Parents who require support to maintain or improve their family life.

  • People who require specialist support with issues such as debt, welfare, housing or homelessness.


Exclusion Criteria

Please note that whilst the service is open to anybody aged 18+, we are unable to support anyone with:

  • On-going personal assistance or support.

  • Currently residing in a residential / care home.

  • With unmanaged mental health diagnoses.

  • Whose health and social care needs are of a severity deemed in need of a professional health and social care consultation.

  • Under the age of 18.

  • Who has not given informed consent to this service.

  • Who displays unreasonable behaviour unacceptable to the provider and its staff


The Social Prescribing Service is primarily a signposting service that will direct and refer individuals to the relevant and more specialist support services. We aim to be the bridge that allows individuals to have knowledge of and access to the variety of services available to them. We do not provide long-term support, and once the referral has been worked on, it will be closed. Patients can re-refer back into the service if they require Social Prescribing support at any point in the future.

Crisis Line

Open 365 days a year, 24 hours a day for mental health advice and support. Anyone can call and no appointment is needed.

Safe Havens

Safe Havens provide out-of-hours help and support to people who are experiencing a mental health crisis or emotional distress and their carers. The Safe Havens are open evenings, weekends and bank holidays. The Safe Havens are designed to provide adults with a safe alternative to A&E when in crisis.
